Liberty Summer Child Development Program


The Office of Child Development is currently accepting applications for the Liberty Summer Child Development Program.  The Liberty Summer Child Development Program provides summer childcare and enrichment for students enrolled in grades K-6th.  This program is a state-subsidized program that provides financial assistance to families who qualify.  Families must meet need and income eligibility requirements to enroll.  Childcare services are only provided during the hours parents are at work and/or school. 


Families must meet the following requirements to enroll: 
  • Child must be enrolled in grades K-6th
  • Childcare is only provided for low-income parents who work or go to school
  • Proof of residency (copy of bill showing 9-digit zip code)
  • Proof of income
  • Proof of need
  • Copy of birth certificates for every child in the home, under the age of 18 
  • Legal notice of Separation or Divorce and court order stipulating child support, if applicable 
  • Additional requirements for parents in school or training

The Office of Child Development operates eight on-campus, before and after school childcare programs; two before and after school programs for low-income families and one full day state preschool.

The Office of Child Development is a FRAGRANCE-FREE ENVIRONMENT.  If planning a visit to our Office, please refrain from wearing any scented product; including, but not limited to, perfumes, colognes and lotions.  If you arrive at our Office wearing any type of scented product, you will be asked to step outside.  Thank you for your consideration in this matter.

Before & After School Childcare/Enrichment

PLEASE CONTACT THE OFFICE OF CHILD DEVELOPMENT REGARDING FUN CLUB SPACE AVAILABILITY FOR 2014-2015.  Limited space is still available at some Fun Club locations.  The Office of Child Development can not guarantee space availability.  Space is offered on a first come, first serve basis.  When childcare space is not available at a program, families are wait listed in the order in which they contact the Office of Child Development.

If space is available at your Fun Club, a fully completed enrollment packet must be submitted in person to the Office of Child Development.  We are located in Room 17 on the Adult School Campus at 12970 Third Street, in Chino.  Please park in the parking located on Monte Vista Avenue.  You will need to walk across the black top area to the last row of classrooms back-to-back with Riverside Drive.  Please call in advance when planning a visit to our Office.

Please bring your current picture identification card or driver's license with you.  Payment of the enrollment fee and first week's tuition is REQUIRED on the day you submit your enrollment packet.  

Please refrain from wearing any scented product when visiting our Office.  If you arrive wearing any scented product, you will be asked to step outside.  Scented products are FATAL to some staffs.

FUN CLUB HOURS OF OPERATION

All programs must maintain sufficient enrollment to fully fund the cost of Fun Club operations.  When a Fun Club location fails to fully fund the cost to support its operation, whether in all or part of the program, action may be taken to close or modify the program. 

Each Fun Club's hours of operation are based on current enrollment and school bell schedule.  The hours of operation is subject to change whenever there is a change in school bell schedule and/or a significant drop in enrollment.

FUN CLUB OPERATIONAL DAYS

Fun Club operates according to the Chino Valley Unified School District School Calendar.  Fun Club is closed on all non-school days.  Unless otherwise noted, all Fun Clubs will be CLOSED on the following days during the 2013-2014 Fun Club contract year:

Monday, November 11, 2013 - Veteran's Day
Monday, January 20, 2014 - Martin Luther King Day
Monday, February 10, 2014 - Lincoln's Birthday
Monday, February 17, 2014 - Washington's Birthday

The Office of Child Development DOES NOT adjust tuition for the above listed days Fun Club is closed.  All families are required to pay their FULL weekly tuition for the above listed weeks.


Unless otherwise noted, all Fun Clubs will be CLOSED on the following Breaks during the 2013-2014 Fun Club contract year:

Monday-Friday, November 25-29, 2013 - Thanksgiving Break
Monday-Friday, December 23, 2013-January 3, 2014 - Winter Break

Monday-Friday, March 31-April 4, 2014 - Spring Break

Tuition is not required for the above listed Breaks Fun Club is closed.


The last day of Fun Club for the 2013-2014 school year is the last day of school, Thursday, June 6, 2014. 
Tuition for the last week of school will be adjusted for families contracted for 5-days per week.  If your child will not be attendance at Fun Club the last week of school, an Absence Notice MUST be submitted by Monday, May 26th.

Liberty & Newman Child Development Centers 
Chino Children's Center


Liberty & Newman Child Development Centers: The Child Development Centers provide before, after school and summer childcare and enrichment for students enrolled in grades K-6th.  At the Child Development Centers, children participate in enrichment activities, are provided with homework assistance, enjoy a nutritious snack and have time to play with and make new friends! 

Before and after school childcare is available at the Newman and Liberty Child Development Centers.  Summer childcare is available only at the Liberty Child Development Center. 

Families must meet the following requirements to enroll:
 

  • Child must be enrolled in grades K-6th
  • Childcare is only provided for low-income parents who work or go to school

Chino Children's Center:  The Chino Children's Center is a full-day state preschool program.  The Chino Children's Center is located on the Adult School Campus, in the city of Chino.  The preschool instructional program includes age-appropriate curriculum with a variety of developmental and enrichment activities to foster growth in the following areas of the child's development:  cognitive, physical and language development; creative, nutritional, self-help and social skills.

Families must meet the following requirements to enroll:

  • Child must be 3-4 years of age
  • Childcare is only provided for low-income families who work or go to school
  • Proof of residency (copy of bill showing 9-digit zip code)
  • Proof of income
  • Proof of need
  • Copy of birth certificate for every child in the home, under the age of 18
  • Updated immunization records for every child enrolled
  • Legal notice of Separation or Divorce and court order stipulating child support, if applicable
  • Additional requirements for parents in school or training

SOAR (Students On A Rise)
After School Academic Program
1st-6th Grade

SOAR (Students On A Rise) is an after school academic program located on the Anna Borba, Levi Dickey, Doris Dickson, Alicia Cortez, EJ Marshall and Walnut Elementary School campuses.  SOAR is not a child care program.  It is a voluntary 5-day a week after school academic program for students in grades 1st-6th who attend the above listed schools.  Pick-up time for all students is 6:00 PM.  SOAR is closed on all non-school days.   

SOAR is currently accepting applications for the 2014-2015 lottery.  Enrollment packets must be completed and submitted to your preferred SOAR program school site by April 30th to be entered in the lottery.

After School Program Proposed at Magnolia Jr. High School for Fall 2014

The Chino Valley Unified School District, in partnership with the Focus on Youth Collaborative, City of Chino, Chino Valley YMCA and other local businesses and non-profit agencies, will apply for a grant to fund an after school program at Magnolia Jr. High beginning in the fall of 2014. The $150,000.00 annual appropriation for the program is funded through the California State “21st Century Community Learning Centers Program.” The goals of the after school center will be to: provide students with academic enrichment including homework assistance and tutoring; offer students a variety of additional activities, such as drug prevention, character building , art, music, sports, and career education; and offer families of participating students opportunities for literacy development. Students, parents, teachers, and community members will be included in planning the program and activities. The free program will open at the end of the school day and will be open until 6:00 p.m. on all 180 school days. Notification of the grant awards will be made by June 2014.

FUN CLUB FORMS AND INFORMATION
TUITION PAYMENTS

Tuition must be paid IN ADVANCE.  All tuition payments are due the Thursday before the upcoming week of childcare, whether the child is present or not.

The Fun Club and the Office of Child Development staff are not responsible for reminding parents of their obligation to keep their account current and to make timely tuition payments.  Parents are responsible to ensure their account remains current at all times.

  • All tuition payments must be paid in full by the Thursday before the upcoming week of childcare.
  • When the weekly tuition fee is not received in full by Thursday, the account will be automatically charged a $25 penalty fee.
  • Accumulation of five (5) late payments within a contract year will be grounds for termination from the program.
  • If your account becomes one week past due, your child may be IMMEDIATELY TERMINATED from the program.
  • Tuition may be paid by check or money order at the Fun Club.  Cash payments are ONLY accepted at the Office of Child Development.
  • Make checks payable to CVUSD or Chino Valley Unified School District.
  • The Office of Child Development does not honor post dated checks.

ABSENCE NOTICE
DEADLINE:  The Monday before the week of absence.

When your child will be absent from Fun Club for a full week, you may be eligible to receive tuition credit by using the Absence Notice form.

The following conditions must be met to receive credit:
  • The Absence Notice must be received in writing on or before the Monday before the upcoming week of absence
  • The Absence Notice must be for a full week (Monday-Friday)
  • There is a limit of two (2) absence weeks per child, per contract year
  • The Absence Notice can not exceed the maximum allowed per child, per contract year

CHANGE OF CONTRACT

DEADLINE: The Monday before the week of change.

To alter an existing Enrollment Contract, a written Change of Contract is required. All Change of Contracts are due the Monday before the upcoming week of change. There is a $25 fee for all Change of Contracts and a $20 fee for all Change of Contracts received after the stated deadline.

A Change of Contract is required to:
  • Add/Delete Days: i.e. your child attends 3 days after school only and you now need them to attend 5 days after school)
  • Add Unplanned/Extra Minimum Days: i.e. Parent Conference Week
  • Change Days of Attendance: i.e. your child attends Monday and Tuesday and you now need them to attend Wednesday and Thursday.
  • Add a Sibling: in addition, a Child Saftey Information and Homework Contract must be completed. Applicable enrollment fees will apply.
Your Change of Contract can be faxed to the Office of Child Development at (909) 548-6090 or emailed to hannah_alvarez@chino.k12.ca.us.

TERMINATION OF ENROLLMENT

DEADLINE: Two weeks before the final date of attendance.

To terminate your child's enrollment at the Fun Club, a Termination of Enrollment Contract must be completed.  All notices must be received in writing the Monday before the upcoming week of termination--two full weeks in advance of your child's final attendance date.

The final two weeks of attendance must be paid at your contracted tuition rate. An Absence Notice will not be honored in lieu of or in combination with your termination notice in an effort to avoid your two full weeks' advance notice requirement.

EMERGENCY DAY

An Emergency Day is any non-contracted day or time of attendance.  The Emergency Day fee is $35 per child, per Emergency Day.

Example 1:
  If you child's contracted days of attendance are Monday and Tuesday, but he/she comes on Wednesday also; Wednesday would be deemed an Emergency Day.

Example 2:  If your child is contracted for 5-days after school only, but he/she comes to Fun Club before school on Monday; Monday before school would be deemed an Emergency Day.

You MUST contact the Fun Club PRIOR to using an Emergency Day.  A child who shows up at the Fun Club without prior notification will be escorted to the school office.

 Child Development Programs
The Office of Child Development operates 3 state-subsidized Child Development Programs: Chino Children's Center, Libery & Newman Child Development Centers and Buena Vista Teen Parent Infant/Toddler Center.

The Chino Children's Center is a full day state preschool, for children 3-4 years old. The Children's Center is located on the Adult School Campus at 12970 Third Street, in the city of Chino.

The Child Development Center is a before/after and summer childcare and enrichment program for kindergarten thru 6th grade students. Child Development Centers are located on Newman and Liberty Elementary School campuses.

The Teen Parent Infant/Toddler Center is located at Buena Vista High School and provides early care and education for children of teen parents enrolled in and attending a CVUSD school. The center provides care for children 6 weeks to 3 years old.

Families must meet income and need criteria to qualify for the Chino Children's Center or the Child Development Program.
