To alter an existing Enrollment Contract, a written Change of Contract is required. All Change of Contracts are due the Monday before the upcoming week of change. There is a $25 fee for all Change of Contracts and a $20 fee for all Change of Contracts received after the stated deadline.

To terminate your child's enrollment at the Fun Club, a Termination of Enrollment Contract must be completed. All notices must be received in writing the Monday before the upcoming week of termination--two full weeks in advance of your child's final attendance date. The final two weeks of attendance must be paid at your contracted tuition rate. An Absence Notice will not be honored in lieu of or in combination with your termination notice in an effort to avoid your two full weeks' advance notice requirement. EXIT QUESTIONNAIREUpon termination from the Fun Club, please help us improve program quality by completing the Fun Club Parent Exit Questionnaire. All questionnaires remain anonymous. Your Exit Questionnaire can be faxed to the Office of Child Development at (909) 548-6090 or emailed to hannah_alvarez@chino.k12.ca.us. 2011-2012 Exit Questionnaire.pdf EMERGENCY DAYAn Emergency Day is any non-contracted day or time of attendance. The Emergency Day fee is $35 per child, per Emergency Day. Example 1: If you child's contracted days of attendance are Monday and Tuesday, but he/she comes on Wednesday also; Wednesday would be deemed an Emergency Day. Example 2: If your child is contracted for 5-days after school only, but he/she comes to Fun Club before school on Monday; Monday before school would be deemed an Emergency Day. You MUST contact the Fun Club PRIOR to using an Emergency Day. A child who shows up at the Fun Club without prior notification will be escorted to the school office.
